Diarrhea is common in long term care hospitals but at many times get underdiagnosed. Symptomatic treatment of diarrhea is sometimes effective but tends to relapse after treatment is seized and also side effects are seen. The root cause of diarrhea must be evaluated in order to treat diarrhea effectively and efficiently. Here are some commonly encountered causes of diarrhea in long term care hospitals and how to approach.

Fecal Calprotectin in Children Can Differentiate between Different Gastrointestinal Diseases

1. Abstract

1.1. Background: Fecal calprotectin (FC) is increased in gastrointestinal inflammation due to increased permeability of the gastrointestinal mucosa. It could be used to differentiate between different gastrointestinal disorders.

1.2. Purpose: The aim of the present study was to examine the relationship between fecal calprotectin and different pediatric gastrointestinal diseases and to investigate if it could be helpful in differentiation between different GI diseases.

Three Different Presentations of Metastatic Gastric Melanoma

1. Abstract

Melanoma is the most common metastatic tumor of the gastrointestinal (GI) tract and its diagnosis is rare during life since its symptoms are non-specific. The endoscopic gastric metastases appearance is heterogenous with no typical pattern. We report a patient who presented three different morphologies of metastatic gastric melanoma although the patient remained asymptomatic. This finding shows the importance of early upper digestive endoscopy in patients diagnosed with melanoma even in the absence of gastrointestinal symptoms.

Intestinal Ischemia Following Cardiac Surgery: An Unusual Clinical Presentation

1. Abstract

Mesenteric ischemia is a dreadful complication with a high mortality rate. Early diagnosis and management is mandatory to improve prognosis.

We report a 72-year-old woman who has undergone a bioprosthetic aortic valve replacement. She presented an abdominal pain associated to an occlusive syndrome. Angio-CT scan has found initially a functional bowel obstruction without mesenteric ischemia. 3 days later, she became febrile and her abdominal distension increase. The fellow up Angio-CT scan revealed a pneumoperitoneum suggestive of a bowel perforation. A diagnostic laparotomy had found two ischemic ileal perforations. The ischemic origin was confirmed by histo-pathological examination. She underwent segmental ileal resection with end-to-end anastomosis. The follow up was favourable.

Background: Fistula in-ano is an abnormal chronic infected tunnel ( tract ) between the rectum or the anal canal and perianal skin usually ,with granulation tissue which connecting a primary orifice ( internal ) to a secondary one ( external) .This morbid antity represents a dilemma for both ,patients and surgeon-proctologists because of postoperative issues occurrence such as fecal incontinence .The surgical management of this infectious condition has emerged nowadays ,by emphasing new surgical techniques that preserve anal sphincter’s integrity and functional Over the last decade, numerous techniques sparing anal sphinters have gained popularity such as endoscopic approach ( VAAFT ), Laser, fibrin glue, transanal advancement flap repair [1, 6]. In the same perspective, we are presenting in this paper, our preliminary outcomes of a combination of endoscopic approach (VAAFT) with Radifrequency Ablation of the fistula’s tract that we nicknamed with the acronym of VAAFT-FisTura).
